As part of its Corporate Citizenship program, Zoetis is making available certain products as a charitable donation to the Association of Zoos and Aquariums that will assist in meeting critical medical and health needs of animals.
Eligibility
• Donated product is available only to 501(c)(3) non-profit AZA-accredited zoos and aquariums (or through the zoo or aquarium's associated 501(c)(3) non-profit organization) and to 501(c)(3) non-profit certified related facilities
• Requesting organization must be located within the U.S.A.
How to Enroll
Fill out the Zoetis Registration Form and return to AZA with a copy of your IRS Determination Form.
Designate Your Authorized Licensed Veterinarian
Use the registration form to designate your authorized licensed veterinarian. Only a licensed veterinarian may make product donation requests. Please share your IN-or RF-level login to the AZA website with your designated veterinarian. If you need assistance with your login, contact Barb Skewes, AZA Membership Coordinator, bskewes@aza.org.
